AT THE TIME OF YOUR INTERVIEW, YOU ARE REQUIRED TO BRING ANY/ALL OF THE FOLLOWING INFORMATION, WHICH APPLIES, TO YOU OR ANY MEMBER OF YOUR FAMILY. ALL ADULT MEMBERS OF THE HOUSEHOLD, 18 YEARS OR OLDER, MUST ATTEND THE ELIGIBILITY PREVIEW APPOINTMENT. PLEASE FOLLOW INSTRUCTIONS IN THE APPROPRIATE AREAS BELOW TO DETERMINE WHAT INFORMATION YOU MUST PROVIDE AT THE TIME OF YOUR MEETING.

APPLICATION FEE: A $25.00 application fee is required at the time of your appointment.

REQUIRED INFORMATION:

1. Picture I.D.

2. $25 Application Fee.

3. Copy of Federal Tax Returns for the last two years.

4. EMPLOYMENT: A Letter on company letterhead including: date of hire, hours worked per week and hourly rate of pay or monthly salary. Also please bring your most recent pay stubs for the last two months (if your earnings vary, we will need to see pay stubs for a longer period).

5. SOCIAL SECURITY CARDS: Social Security cards are required for all family members.

6. ASSETS: Including savings, checking accounts, C. D.’s, Stocks, Bonds, Notes, and Mortgages held, and Trusts. Please bring the three most recent bank statements or letters from the financial institution(s) confirming account number(s), balance(s), and rate(s), of interest paid.

7. BIRTH CERTIFICATES: Birth Certificates must be provided for all family members under the age of 18. Baptismal Certificates are also acceptable. Legal guardianship papers must be provided for any other children.

OPTIONAL INFORMATION BASED ON YOUR HOUSEHOLD:

  1. SELF-EMPLOYMENT: Copy of Federal Income Tax return, showing income earned and business deductions (Schedule C), or signed dated statement from accountant containing gross income, expenses, net profit, and business location plus Year-To-Date statement of income and expenses for the current year.

  1. UNEMPLOYMENT: Document which shows gross benefit amount received.

  1. WELFARE ASSISTANCE: Please bring your most recent award letter, showing current amount received.

  1. SOCIAL SECURITY, SUPPLEMENTAL SECURITY INCOME (SSI) AND PENSIONS: Bring most recent Social Security award letters or print-outs from Social Security Administration for all family members receiving benefits.

  1. VETERAN BENEFITS: Current Veterans benefit award letter.

  1. CHILD SUPPORT: A print-out or letter is needed from the Child Support Division from which child support payments are received detailing payments currently being made. If not received from Child Support Division, please supply us with a notarized letter, giving details of payments, from child support provider.

  1. REAL PROPERTY: Name, address, and zip code of where current market value of the property can be obtained.

  1. DIVORCE DECREES: A copy of the legal divorce decree(s) is needed.

  1. LOANS: Please bring information about outstanding loan(s), the amount(s) owed; date the loan(s) was taken out, monthly payments due, and the number of payments.

  1. OTHER: Any additional information determined to be necessary to complete your eligibility review must be provided.
